under the terms of Art. 6 para. 1 letter a) and Art. 7 Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) and Act no. 18/2018 Coll. on the protection of personal data and on the amendment of certain laws.

I hereby give my consent to the processing of my personal data under the Personal Data Protection Act, as amended. The consent is granted to AspenStaff, s.r.o., based in Klariská 7, 811 03 Bratislava, for the purposes of employment mediation and securing documentation related to it, obtaining information about suitable job opportunities and other services related to career and personal development. For this purpose, I am in favour of assigning to candidate database of employment operator and following provision or accessing my personal data to prospective employments, to the extent of:

  • Name, surname, title,
  • Date and place of birth, gender, ID number, citizenship, nationality, marital status,
  • Permanent and temporary residence (name and number of street, building number, town, postal code),
  • Contact details (e-mail, phone number),
  • Bank account details, health insurance company, photography,
  • Labor relation (current and previous employment information, to the extent of employer 's address and occupation),
  • Educational attainment,
  • Data on completed courses and trainings,
  • Information about health status provided on a voluntary basis,
  • Data on personal and family relationships related to the process of employment mediation,
  • Other information included in CV.

I hereby declare that I have been provided with the following information related to Personal Data processing before giving my consent:

  • My personal data will be provided to the recipients (the potential employers and clients of AspenStaff, s.r.o.).
  • Personal data I have provided to AspenStaff, s.r.o. on a voluntary basis will be stored by the time of validity of this consent and will not be processed for any other purpose. 
  • As a person concerned, I have the right to request from the AspenStaff, s.r.o. access to my Personal Data, the right to repair, delete or limit my Personal Data, the right to limit the processing of Personal Data, the right to object to the processing of Personal Data and the right to the portability of Personal Data.
  • I note that there is no automated decision making, including profiling.
  • I have the right only in the justified case to file a grievance with the supervisory authority, which is the Office for Personal Data Protection of the Slovak Republic

The consent to the processing of personal data is valid for a fixed period of five years, but it may be revoked at any time.
