I hereby grant (hereinafter referred to as “Data Subjects”) the company HOFMANN WIZARD, sro, with its registered office at Prešovská 201/4, Plzeň, Postal Code 301 00, Company Identification Number: 252 36 016, entered in the Commercial Register kept by the Regional Court in Pilsen, Section C, Insert 11501 , email contact: kontakt@hofmann-personal.cz (hereinafter referred to as the "Administrator"), as the employment agency and the administrator of personal data consent to the processing of my personal data, under the following conditions:

  1. Personal data to be processed:
    Name and surname, mailing address, e-mail address, phone number, date of birth; Other personal data resulting from the Subject´s curriculum vitae: date and type of studies, academic degrees, previous work experience and its duration, language skills, technical skills, etc.
  2. Purpose of the personal data processing is:
    The processing of personal data is carried out in order to fulfill a contract between the Administrator and the Data Subject. Based on this contract the Administrator ensures to the Data Subject the possibility to apply for a job at the Administrator, in other words the possibility of temporary assignment to the User, the possibility to apply for a job at another employer and, where applicable, the possibility to conclude a contract/agreement for work done beyond the employment relationship and for the purpose of protecting the Administrator´s legitimate interests. The personal data processing is also carried out for the purpose of entry in the register of job seekers maintained by the Administrator and for the purpose of satisfying the Data Subject´s interest in being sent job offers from the Administrator to which the Data Subject has given explicit consent.
  3. Duration of the personal data processing is:
    Personal data will be processed and stored for a period of five years from the consent to its entry in the register of job seekers, or until withdrawal of such a consent. After this period, the personal data will be irretrievably destroyed.
  4. Personal data may be provided to the following third persons, for the purpose of further processing by the Administrator or for the purpose of selecting a future employee:
    • Information system provider for the recruitment management purposes.
    • Wage, accounting and tax services provider.
    • Parent company, information system provider.
    • Users who are interested in finding a suitable employee assigned to them under a temporary assignment agreement
    • Administrator´s customers interested in finding a suitable employee.
    • Providers of electronic document signing services, for the purposes of identification of the signatory and administration of signed documents.
    • Providers of consultancy services in the field of quality management and whistleblowing.

In accordance with provisions of article 13 of the European Parliament and Council Regulation (EU) No. 2016/679 of 27 April 2016 - General Regulation on the protection of personal data (hereinafter referred to as "the Regulation"), the Administrator hereby informs that:

  • The Administrator has not appointed any Data Protection Officer.
  • The Administrator does not intend transferring the Data Subject´s personal data to any third country, international organization or other than the above third parties.
  • The Data Subject has the right at any time to withdraw its consent to the entry to the register of job seekers.
  • The Data Subject has the right to require from the Administrator accessing, correcting or deleting his/her personal data, limiting or objecting its processing, furthermore he/she has the right to transfer these data to another administrator, as well as the right to file a complaint with the Personal Data Protection Authority, when considering that the Administrator is in breach of the Regulation in the processing of personal data.
